Narrative:

On the morning of mar/xa/00 after doing the entire portion of an 'a' check on aircraft xyz I found 3 main wheel assemblies with sidewall wear beyond limits. Another mechanic and I replaced all 3 tires. All 3 tires were changed per the maintenance manual -- torqued, spun freely after torquing with no side play. The aircraft later went out for a maintenance run where the #7 tire locked up due to the gear spacer being missing. I signed the confirmation for the changing of this wheel assembly.

Title: A B767 ON CHK MAINT RUN EXPERIENCED #7 WHEEL LOCKED DUE TO FAILURE TO INSTALL THE WHEEL SPACER ON REPLACEMENT OF THE #7 WHEEL AND TIRE ASSEMBLY.

THE ACFT LATER WENT OUT FOR A MAINT RUN WHERE THE #7 TIRE LOCKED UP DUE TO THE GEAR SPACER BEING MISSING. I SIGNED THE CONFIRMATION FOR THE CHANGING OF THIS WHEEL ASSEMBLY.
