Narrative:
On oct/mon/05; I was involved working a triple tire change on air carrier X on a through flight; epdc-1 inspection. 2 main tires I found worn or cut beyond service limits and the nose tire (left) position was on maintenance carryover for wear. Working with 2 other amt's; all 3 tires were replaced and documented in the aircraft logbook. I have learned today (oct/fri/05) that the spacer on the left nosewheel was not installed. This was discovered at a station where a replacement spacer was not available and the aircraft; after proper documentation; was allowed one landing to a station and a new spacer was installed. The missing spacer went unnoticed by 3 sets of eyes of 15+ (yrs) each line amt's. Factors that may have caused this mistake are: complacency. Tire changes are very common and it's easy to drop your guard and not scrutinize each and every step of such a common task. It's also very possible that the spacer was not there in the first place due to the fact that one was not found with the removed wheel assembly. This would greatly contribute to the oversight; and I believe this is what happened.

Title: A B767-300ER WAS DISPATCHED WITH THE L NOSE TIRE AXLE SPACER MISSING. TIRE AND WHEEL ASSEMBLY REPLACED ON OVERNIGHT MAINT.
Narrative: ON OCT/MON/05; I WAS INVOLVED WORKING A TRIPLE TIRE CHANGE ON ACR X ON A THROUGH FLT; EPDC-1 INSPECTION. 2 MAIN TIRES I FOUND WORN OR CUT BEYOND SVC LIMITS AND THE NOSE TIRE (L) POS WAS ON MAINT CARRYOVER FOR WEAR. WORKING WITH 2 OTHER AMT'S; ALL 3 TIRES WERE REPLACED AND DOCUMENTED IN THE ACFT LOGBOOK. I HAVE LEARNED TODAY (OCT/FRI/05) THAT THE SPACER ON THE L NOSEWHEEL WAS NOT INSTALLED. THIS WAS DISCOVERED AT A STATION WHERE A REPLACEMENT SPACER WAS NOT AVAILABLE AND THE ACFT; AFTER PROPER DOCUMENTATION; WAS ALLOWED ONE LNDG TO A STATION AND A NEW SPACER WAS INSTALLED. THE MISSING SPACER WENT UNNOTICED BY 3 SETS OF EYES OF 15+ (YRS) EACH LINE AMT'S. FACTORS THAT MAY HAVE CAUSED THIS MISTAKE ARE: COMPLACENCY. TIRE CHANGES ARE VERY COMMON AND IT'S EASY TO DROP YOUR GUARD AND NOT SCRUTINIZE EACH AND EVERY STEP OF SUCH A COMMON TASK. IT'S ALSO VERY POSSIBLE THAT THE SPACER WAS NOT THERE IN THE FIRST PLACE DUE TO THE FACT THAT ONE WAS NOT FOUND WITH THE REMOVED WHEEL ASSEMBLY. THIS WOULD GREATLY CONTRIBUTE TO THE OVERSIGHT; AND I BELIEVE THIS IS WHAT HAPPENED.
