Narrative:
C182 departed cdw on an IFR flight plan to orf, cleared to 5000 ft, radar vector dixie. When reaching 5000 ft, pilot set altitude hold, but failed to monitor. Climb continued to 5400 ft. Controller queried 'did I clear you to 6000 ft? (Final altitude)?' response 'negative, 5000 ft' and immediately return to 5000 ft. Controller response 'no problem, 12 mi separation.' probable cause: daughter with serious health problem in orf. Passenger (wife) distraught and upset -- flight to assist daughter. Recommendation: greater concentration and disable intercom.

Narrative: C182 DEPARTED CDW ON AN IFR FLT PLAN TO ORF, CLRED TO 5000 FT, RADAR VECTOR DIXIE. WHEN REACHING 5000 FT, PLT SET ALT HOLD, BUT FAILED TO MONITOR. CLB CONTINUED TO 5400 FT.
