Narrative:

After being pushed back from the gate, we continued our normal ground checks and taxi procedures. The first officer received the taxi clearance and we continued. Ground control was very busy and there was a line forming for the runway in use. As we continued our taxi and preflight checklist, ground control asked where we were. Then they saw us and said that we were supposed to have held short of taxiway D. Both the captain and first officer said they never heard that and they did not read it back with the taxi instructions. As the first officer, I never heard the hold short of taxiway D clearance for us. We proceeded in line for the runway and nothing else was said. There was not a conflict between our aircraft and any other on the ground. There was a miscom between ground control and our aircraft. We never heard the hold short instruction and ground thought that they gave it to us. The miscom could have been caused by heavy ground frequency talk, very busy ground controller, and very busy cockpit crew doing preflight checks.
