Primary Problem | Flight Crew Human Performance |
Air Traffic Incident | Pilot Deviation |
Narrative:
While repositioning aircraft, the crew was issued the clearance 'taxi via taxiway west to taxiway J....' while taxiing, the crew made a wrong turn and nearly crossed runway 5R/23L (active). Crew interaction along with ATC direction to stop and turn around averted a potential runway incursion. Contributing factors: 1) poor taxiway markings and layout. 2) poor airport diagram depiction of txwys at that area. 3) inadequate review of airport diagram. 4) low light (night). 5) inadequate taxi light. The failure of the crew to follow ATC instructions was caused by unclr taxi instructions and inadequate review of appropriate airport diagram to familiarize the confusing taxiway intersection. Callback conversation with reporter revealed the following information: reporter said that the area is wide open with only taxi lines defining the txwys. He was using the commercial chart but admitted to an inadequate review.
Original NASA ASRS Text
Title: TXWY INCURSION AT CLEVELAND HOPKINS ARPT. COMPLAINT OF TAXI LINE MARKINGS CONFUSION.
Narrative: WHILE REPOSITIONING ACFT, THE CREW WAS ISSUED THE CLRNC 'TAXI VIA TXWY W TO TXWY J....' WHILE TAXIING, THE CREW MADE A WRONG TURN AND NEARLY CROSSED RWY 5R/23L (ACTIVE). CREW INTERACTION ALONG WITH ATC DIRECTION TO STOP AND TURN AROUND AVERTED A POTENTIAL RWY INCURSION. CONTRIBUTING FACTORS: 1) POOR TXWY MARKINGS AND LAYOUT. 2) POOR ARPT DIAGRAM DEPICTION OF TXWYS AT THAT AREA. 3) INADEQUATE REVIEW OF ARPT DIAGRAM. 4) LOW LIGHT (NIGHT). 5) INADEQUATE TAXI LIGHT. THE FAILURE OF THE CREW TO FOLLOW ATC INSTRUCTIONS WAS CAUSED BY UNCLR TAXI INSTRUCTIONS AND INADEQUATE REVIEW OF APPROPRIATE ARPT DIAGRAM TO FAMILIARIZE THE CONFUSING TXWY INTXN. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: RPTR SAID THAT THE AREA IS WIDE OPEN WITH ONLY TAXI LINES DEFINING THE TXWYS. HE WAS USING THE COMMERCIAL CHART BUT ADMITTED TO AN INADEQUATE REVIEW.
