Narrative:
Radio altimeter, TCASII, GPWS were all deferred in accordance with MEL due to a flagged radio altimeter. Dispatcher amended the release via radio. It was discovered by another crew after 4 legs that the captain did not add the deferrals in the aircraft logbook. This crew flew the aircraft 4 legs before noticing it. I dispatched the last leg of this trip!

Title: DC9 CREW DID NOT ADD DEFERRED MEL ITEMS TO THE ACFT LOGBOOK.
Narrative: RADIO ALTIMETER, TCASII, GPWS WERE ALL DEFERRED IN ACCORDANCE WITH MEL DUE TO A FLAGGED RADIO ALTIMETER. DISPATCHER AMENDED THE RELEASE VIA RADIO. IT WAS DISCOVERED BY ANOTHER CREW AFTER 4 LEGS THAT THE CAPT DID NOT ADD THE DEFERRALS IN THE ACFT LOGBOOK. THIS CREW FLEW THE ACFT 4 LEGS BEFORE NOTICING IT. I DISPATCHED THE LAST LEG OF THIS TRIP!
