Narrative:
Contract mechanic called to placard aircraft xyz. He requested to placard #1 reverser unlock lights. I advised him that in addition to that, we would need to deactivate and placard #1 reverser as well, and the 2 references would be abc for the lights and cba for the #1 reverser inoperative. I told him that is what air carrier recommends. I asked him if he was comfortable with deactivating the lights and reverser inoperative. He said he was ok with it. I issued the 2 placards and asked again if he was ok with it -- and again he said he was ok with it. The aircraft left ZZZ to XXX with the reverser not deactivated and lights not deactivated per MEL.

Title: AN AIRBUS A300 WAS DISPATCHED IN NON COMPLIANCE WITH THE #1 ENG REVERSER AND REVERSER UNLOCK LIGHT DEFERRED AS INOP BUT MEL SPECIAL PROCS WERE NOT ACCOMPLISHED.
Narrative: CONTRACT MECH CALLED TO PLACARD ACFT XYZ. HE REQUESTED TO PLACARD #1 REVERSER UNLOCK LIGHTS. I ADVISED HIM THAT IN ADDITION TO THAT, WE WOULD NEED TO DEACTIVATE AND PLACARD #1 REVERSER AS WELL, AND THE 2 REFS WOULD BE ABC FOR THE LIGHTS AND CBA FOR THE #1 REVERSER INOP. I TOLD HIM THAT IS WHAT ACR RECOMMENDS. I ASKED HIM IF HE WAS COMFORTABLE WITH DEACTIVATING THE LIGHTS AND REVERSER INOP. HE SAID HE WAS OK WITH IT. I ISSUED THE 2 PLACARDS AND ASKED AGAIN IF HE WAS OK WITH IT -- AND AGAIN HE SAID HE WAS OK WITH IT. THE ACFT LEFT ZZZ TO XXX WITH THE REVERSER NOT DEACTIVATED AND LIGHTS NOT DEACTIVATED PER MEL.
