Air Traffic Incident | Pilot Deviation |
Narrative:
Off course deviation. At otu 02 bog gave pilot's discretion to FL240. Captain started down and left at FL370 at otu 02. Bog gave us pilot's discretion to FL240. Captain started down and left the cockpit briefly. Bog switched us to 119.5 and we were cleared to bog via mqu uti bog and down to FL220, expect ILS runway 13L. I flipped the altitude selector to FL220 and by now were direct mqu. I then typed in utica and inserted it under mqu in the FMC and line selected bog under utica. Right about this time, the captain returned and we crossed over mqu. I briefed him on the changes and realized that the airplane was in a descending right turn. Uti was a left turn. I reached over and hit heading select to get us going in the right direction and tuned in uti NDB for the raw data information. As we were trying to figure out what had gone wrong, bog cleared us direct bog and down to 12000 ft. The captain figured out that I had entered utica instead of the proper identify uti in the FMC. Bog did not mention the deviation and the flight continued a normal approach and landing in bog.

Title: B757-200 CREW HAD TRACK DEV IN SKED CLASS A AIRSPACE.
Narrative: OFF COURSE DEV. AT OTU 02 BOG GAVE PLT'S DISCRETION TO FL240. CAPT STARTED DOWN AND LEFT AT FL370 AT OTU 02. BOG GAVE US PLT'S DISCRETION TO FL240. CAPT STARTED DOWN AND LEFT THE COCKPIT BRIEFLY. BOG SWITCHED US TO 119.5 AND WE WERE CLRED TO BOG VIA MQU UTI BOG AND DOWN TO FL220, EXPECT ILS RWY 13L. I FLIPPED THE ALT SELECTOR TO FL220 AND BY NOW WERE DIRECT MQU. I THEN TYPED IN UTICA AND INSERTED IT UNDER MQU IN THE FMC AND LINE SELECTED BOG UNDER UTICA. RIGHT ABOUT THIS TIME, THE CAPT RETURNED AND WE CROSSED OVER MQU. I BRIEFED HIM ON THE CHANGES AND REALIZED THAT THE AIRPLANE WAS IN A DSNDING R TURN. UTI WAS A L TURN. I REACHED OVER AND HIT HDG SELECT TO GET US GOING IN THE RIGHT DIRECTION AND TUNED IN UTI NDB FOR THE RAW DATA INFO. AS WE WERE TRYING TO FIGURE OUT WHAT HAD GONE WRONG, BOG CLRED US DIRECT BOG AND DOWN TO 12000 FT. THE CAPT FIGURED OUT THAT I HAD ENTERED UTICA INSTEAD OF THE PROPER IDENT UTI IN THE FMC. BOG DID NOT MENTION THE DEV AND THE FLT CONTINUED A NORMAL APCH AND LNDG IN BOG.
