Narrative:

Our aircraft was pushed back onto an active taxiway. When we contacted ground, they informed us of our incursion and to call the facility. The requirement to contact ground prior to pushback is in our company ramp commercial chart page. The captain and first officer missed the note. Factors in this occurrence: 1) this is the last trip of a month-long crew pairing. The captain has very poor CRM skills. Our cockpit conversation both flight related and general is very short, tense and not conducive to free and open conversation (earlier in the month the captain and first officer had an argument during the climb out phase). 2) as a so, I should be issued airport diagram pages and company ramp commercial pages. This would allow me to know what the airport requirements are.

Narrative: OUR ACFT WAS PUSHED BACK ONTO AN ACTIVE TXWY. WHEN WE CONTACTED GND, THEY INFORMED US OF OUR INCURSION AND TO CALL THE FACILITY. THE REQUIREMENT TO CONTACT GND PRIOR TO PUSHBACK IS IN OUR COMPANY RAMP COMMERCIAL CHART PAGE. THE CAPT AND FO MISSED THE NOTE. FACTORS IN THIS OCCURRENCE: 1) THIS IS THE LAST TRIP OF A MONTH-LONG CREW PAIRING. THE CAPT HAS VERY POOR CRM SKILLS. OUR COCKPIT CONVERSATION BOTH FLT RELATED AND GENERAL IS VERY SHORT, TENSE AND NOT CONDUCIVE TO FREE AND OPEN CONVERSATION (EARLIER IN THE MONTH THE CAPT AND FO HAD AN ARGUMENT DURING THE CLBOUT PHASE). 2) AS A SO, I SHOULD BE ISSUED ARPT DIAGRAM PAGES AND COMPANY RAMP COMMERCIAL PAGES. THIS WOULD ALLOW ME TO KNOW WHAT THE ARPT REQUIREMENTS ARE.
