Narrative:
While acting as sic on a part 135 charter flight, I arrived approximately 1 hour prior to scheduled departure, greeted the captain who at that time was reviewing the flight manifest and completing the weight and balance for the flight. I proceeded to obtain destination WX, preflight checks of the plane, ensured that the fuel load was correct and restocked the aircraft with supplies. Upon arrival in the flight office, I took the flight can and captain's headset and placed it in the aircraft. The flight originated on time and was uneventful during both legs. Upon return to our base, the flight department charter manager questioned us as to the whereabouts of the MEL form 105 and indicated that it was not in the flight can for this flight. Both the captain and I examined the can and found the form in question to be missing. The deferred item was for the oxygen bottle that had been placarded as removed for service. Both the captain and I noted this during our cockpit inspection and assumed that the required paperwork was on board.

Title: A BEECH 30 WAS DISPATCHED IN NON COMPLIANCE WITH NO DOCUMENTATION FOR DEFERRAL OF A REMOVED PORTABLE OXYGEN BOTTLE.
Narrative: WHILE ACTING AS SIC ON A PART 135 CHARTER FLT, I ARRIVED APPROX 1 HR PRIOR TO SCHEDULED DEP, GREETED THE CAPT WHO AT THAT TIME WAS REVIEWING THE FLT MANIFEST AND COMPLETING THE WT AND BAL FOR THE FLT. I PROCEEDED TO OBTAIN DEST WX, PREFLT CHKS OF THE PLANE, ENSURED THAT THE FUEL LOAD WAS CORRECT AND RESTOCKED THE ACFT WITH SUPPLIES. UPON ARR IN THE FLT OFFICE, I TOOK THE FLT CAN AND CAPT'S HEADSET AND PLACED IT IN THE ACFT. THE FLT ORIGINATED ON TIME AND WAS UNEVENTFUL DURING BOTH LEGS. UPON RETURN TO OUR BASE, THE FLT DEPT CHARTER MGR QUESTIONED US AS TO THE WHEREABOUTS OF THE MEL FORM 105 AND INDICATED THAT IT WAS NOT IN THE FLT CAN FOR THIS FLT. BOTH THE CAPT AND I EXAMINED THE CAN AND FOUND THE FORM IN QUESTION TO BE MISSING. THE DEFERRED ITEM WAS FOR THE OXYGEN BOTTLE THAT HAD BEEN PLACARDED AS REMOVED FOR SVC.
