Narrative:

Aircraft returned to the field with the pilot reporting he had lost airspeed and altimeter information on the copilot's side after departure from xyz. Upon examination of the #2 air data computer, I noticed the pitot and static lines were not connected. I re-racked the #2 air data computer and connected the lines. In my haste to return the aircraft to service, I was vague in my signoff. Supplemental information from acn 504392: the cause was loose pitot static lines on air data computer. I understand this is the 4TH return to field due to this cause.

Title: A B737-400 RETURNED TO THE FIELD DUE TO LOSS OF FO'S AIRSPD AND ALTIMETER DATA CAUSED BY PITOT AND STATIC LINES NOT CONNECTED TO THE AIR DATA COMPUTER.

Narrative: ACFT RETURNED TO THE FIELD WITH THE PLT RPTING HE HAD LOST AIRSPD AND ALTIMETER INFO ON THE COPLT'S SIDE AFTER DEP FROM XYZ. UPON EXAMINATION OF THE #2 AIR DATA COMPUTER, I NOTICED THE PITOT AND STATIC LINES WERE NOT CONNECTED. I RE-RACKED THE #2 AIR DATA COMPUTER AND CONNECTED THE LINES. IN MY HASTE TO RETURN THE ACFT TO SVC, I WAS VAGUE IN MY SIGNOFF. SUPPLEMENTAL INFO FROM ACN 504392: THE CAUSE WAS LOOSE PITOT STATIC LINES ON AIR DATA COMPUTER. I UNDERSTAND THIS IS THE 4TH RETURN TO FIELD DUE TO THIS CAUSE.
