Narrative:
On the dallas 6 departure with txk transition from dfw, aircraft was given a heading after takeoff to intercept the departure from the north. Aircraft intercepted and began to track TTT outbound north of transition radial by several degrees. A call was soon received by dfw departure to check radial position. The active waypoint/course was reinserted into FMS and a course correction was made. Cause was that waypoint 'shero' on the departure was not in the active flight plan. FMS preflight procedures were followed. Departure and transition were originally entered, but I believe dropped after a route request/load and not idented during route check. Additionally, checking active waypoint against the SID would have prevented this. Captain was PF and first officer was PNF on first leg of first officer IOE.
Original NASA ASRS Text
Title: S80 FLC ON FO IOE INCORRECTLY INPUT FMC DALLAS SID FROM DFW AND NAV ERROR CAUGHT BY D10 CTLR.
Narrative: ON THE DALLAS 6 DEP WITH TXK TRANSITION FROM DFW, ACFT WAS GIVEN A HDG AFTER TKOF TO INTERCEPT THE DEP FROM THE N. ACFT INTERCEPTED AND BEGAN TO TRACK TTT OUTBOUND N OF TRANSITION RADIAL BY SEVERAL DEGS. A CALL WAS SOON RECEIVED BY DFW DEP TO CHK RADIAL POS. THE ACTIVE WAYPOINT/COURSE WAS REINSERTED INTO FMS AND A COURSE CORRECTION WAS MADE. CAUSE WAS THAT WAYPOINT 'SHERO' ON THE DEP WAS NOT IN THE ACTIVE FLT PLAN. FMS PREFLT PROCS WERE FOLLOWED. DEP AND TRANSITION WERE ORIGINALLY ENTERED, BUT I BELIEVE DROPPED AFTER A RTE REQUEST/LOAD AND NOT IDENTED DURING RTE CHK. ADDITIONALLY, CHKING ACTIVE WAYPOINT AGAINST THE SID WOULD HAVE PREVENTED THIS. CAPT WAS PF AND FO WAS PNF ON FIRST LEG OF FO IOE.
