Narrative:
C172, aircraft X, on vector to downwind runway 23, tower approved minimums. PA28 departed runway 23 into aircraft X, by the tower controller. Radar controller gave emergency vector to aircraft X who barely missed the PA28. Normal procedure when minimums approved by tower is to depart runway heading to avoid downwind traffic. It should be noted that the tower controller was decertified on radar less than 1 hour earlier due to another operational error, and it is this controller's opinion that the tower controller should not have been assigned to work local control so soon after having an error. Management stated that said controller was not being charged with any operrors but they sent him home because he did not seem to be able to concentrate enough to work.

Title: FWA APCH CTLR TURNS INBOUND C172 OFF FINAL APCH WHEN OBSERVING OPPOSITE DIRECTION PA28 ON CLBOUT.
Narrative: C172, ACFT X, ON VECTOR TO DOWNWIND RWY 23, TWR APPROVED MINIMUMS. PA28 DEPARTED RWY 23 INTO ACFT X, BY THE TWR CTLR. RADAR CTLR GAVE EMER VECTOR TO ACFT X WHO BARELY MISSED THE PA28. NORMAL PROC WHEN MINIMUMS APPROVED BY TWR IS TO DEPART RWY HDG TO AVOID DOWNWIND TFC. IT SHOULD BE NOTED THAT THE TWR CTLR WAS DECERTIFIED ON RADAR LESS THAN 1 HR EARLIER DUE TO ANOTHER OPERROR, AND IT IS THIS CTLR'S OPINION THAT THE TWR CTLR SHOULD NOT HAVE BEEN ASSIGNED TO WORK LCL CTL SO SOON AFTER HAVING AN ERROR. MGMNT STATED THAT SAID CTLR WAS NOT BEING CHARGED WITH ANY OPERRORS BUT THEY SENT HIM HOME BECAUSE HE DID NOT SEEM TO BE ABLE TO CONCENTRATE ENOUGH TO WORK.
