Narrative:

I had assigned card xx to the mechanic on apr/xa/01 to do a borescope. The cards were previously racked and the card was in the right engine rack. I was notified later that the wrong engine had been borescoped. Card xx has nowhere on it that idents which engine should be worked. All other engine cards are even numbered for right engine, odd numbered for left. The only way to determine would be to check the bill of work. Supplemental information from acn 509992: I was assigned to do card xx on the r-hand engine. It was in my work package for the r-hand engine. I completed card xx and later learned that card xx was supposed to be completed on the l-hand engine. Nowhere on card xx does it indicate which engine should be accomplished. Supplemental information from acn 510015: I borescoped #2 engine #7 fuel nozzle nut. I should have borescoped #1 engine fuel nozzle nut. Card xx was in with #2 engine paperwork cards and I did not notice a different engine serial number. Supplemental information from acn 509993: I am the third shift supervisor, and when I arrived to work the inspection had been accomplished. There were 2 more accomplishment blocks left on form X. I signed and north/a'ed the block for signing off the accomplishment in the logbook in accordance with gpm. The planner signed off the last block on the form after sending the accomplishment message which had been filled out by the inspector. The borescope had been done on the wrong engine.

SUPPLEMENTAL INFO FROM ACN 509993: I AM THE THIRD SHIFT SUPVR, AND WHEN I ARRIVED TO WORK THE INSPECTION HAD BEEN ACCOMPLISHED. THERE WERE 2 MORE ACCOMPLISHMENT BLOCKS LEFT ON FORM X. I SIGNED AND N/A'ED THE BLOCK FOR SIGNING OFF THE ACCOMPLISHMENT IN THE LOGBOOK IN ACCORDANCE WITH GPM. THE PLANNER SIGNED OFF THE LAST BLOCK ON THE FORM AFTER SENDING THE ACCOMPLISHMENT MESSAGE WHICH HAD BEEN FILLED OUT BY THE INSPECTOR. THE BORESCOPE HAD BEEN DONE ON THE WRONG ENG.
