Narrative:

We flew the modesto 2 arrival and were told to expect ILS runway 28R at sfo. Once on vectors to final (outside of brijj), approach asked us if we could accept the FMS bridge visual runway 28R. We said yes. Approach gave us a heading and told us to fly the published altitude. But, we were not cleared for the approach. Prior to garow, we were given a vector to samul fix. Just prior to samul, we were cleared for the visual runway 28R. The captain descended to 12000 ft MSL and leveled off. Tower notified us we were low on altitude. We acknowledged. We were never cleared for the FMS bridge visual runway 28R approach. Remainder of approach and landing were completed per visual approach rules.

Title: A B767-300 FO RPTED CONFUSION ON THE FMS BRIDGE VISUAL FOR SFO, DURING WHICH THEY DSNDED TOO EARLY.

Narrative: WE FLEW THE MODESTO 2 ARR AND WERE TOLD TO EXPECT ILS RWY 28R AT SFO. ONCE ON VECTORS TO FINAL (OUTSIDE OF BRIJJ), APCH ASKED US IF WE COULD ACCEPT THE FMS BRIDGE VISUAL RWY 28R. WE SAID YES. APCH GAVE US A HDG AND TOLD US TO FLY THE PUBLISHED ALT. BUT, WE WERE NOT CLRED FOR THE APCH. PRIOR TO GAROW, WE WERE GIVEN A VECTOR TO SAMUL FIX. JUST PRIOR TO SAMUL, WE WERE CLRED FOR THE VISUAL RWY 28R. THE CAPT DSNDED TO 12000 FT MSL AND LEVELED OFF. TWR NOTIFIED US WE WERE LOW ON ALT. WE ACKNOWLEDGED. WE WERE NEVER CLRED FOR THE FMS BRIDGE VISUAL RWY 28R APCH. REMAINDER OF APCH AND LNDG WERE COMPLETED PER VISUAL APCH RULES.
