Narrative:
After pushback from gate at hrl, after start check complete. I believe I called for flaps 5 degrees, called for taxi to runway 13 (it's a fairly short taxi). We were in no hurry and made a slow taxi to the runway. We ran the pretkof checklist. We were cleared into position and hold for runway 13 and waited for an arrival on runway 17. Subsequently, we were cleared for takeoff (it was my leg), advanced throttles and got a takeoff warning horn. I hit the speed brake lever to no avail. At that time the first officer pointed at flap position and said 'flaps.' we were barely rolling by then, pulled power to idle and taxied off runway 13 onto taxiway. We told tower what we were doing and said we'd be ready to go in a min. We placed the flaps to 5 degrees, ran the entire checklist, briefed the passenger, and were on our way to aus. We both talked about the event on the way to aus: 1) during announcement of 'after start check complete' (just after), ground gave us taxi instructions. I can't for certain say I called for flaps 5 degrees or not. The first officer said he did not hear me if I did. 2) running checklist, we both failed to catch the flap position. 3) we were not in a hurry/rushing. 4) complacency is always lurking. First day XB30-XC10, 8 legs in a dash 200. Second day 4 legs in a dash 300 arrive maf at XC20. Third day 7 legs in dash 200 and this was #6 of 7 legs. 5) this has never happened in my 24_ yrs here -- why now? 6) I'm a safe pilot, check takeoff and landing confign diligently. Fly by the book! 7) I have no excuse for what happens to us. It's my fault as I am the captain. 8) I'll be as careful as I have been in the past -- even more so!

THE FO SAID HE DID NOT HEAR ME IF I DID. 2) RUNNING CHKLIST, WE BOTH FAILED TO CATCH THE FLAP POS. 3) WE WERE NOT IN A HURRY/RUSHING. 4) COMPLACENCY IS ALWAYS LURKING. FIRST DAY XB30-XC10, 8 LEGS IN A DASH 200. SECOND DAY 4 LEGS IN A DASH 300 ARRIVE MAF AT XC20. THIRD DAY 7 LEGS IN DASH 200 AND THIS WAS #6 OF 7 LEGS. 5) THIS HAS NEVER HAPPENED IN MY 24_ YRS HERE -- WHY NOW? 6) I'M A SAFE PLT, CHK TKOF AND LNDG CONFIGN DILIGENTLY. FLY BY THE BOOK! 7) I HAVE NO EXCUSE FOR WHAT HAPPENS TO US. IT'S MY FAULT AS I AM THE CAPT. 8) I'LL BE AS CAREFUL AS I HAVE BEEN IN THE PAST -- EVEN MORE SO!
