Narrative:
On taxi out from cle when autobrakes were armed, maximum brakes engaged. Flight attendants were doing pretkof checks when brakes locked and were thrown forward. 2 were injured with 1 removing herself from the trip. On taxi in, after landing checks are completed. Either spoiler handle or throttle above idle are supposed to store spoilers. There is no other check that spoilers are stowed. On taxi out, if spoilers are up, they send a signal to the brakes that a rejected takeoff is being executed. The brakes then go to maximum. After the incident, air carrier instituted a new cockpit crew check to include checking the spoilers down. Supplemental information from acn 511941: normal procedures call for first officer to disarm/stow spoilers after landing. If this step is inadvertently missed, spoiler signal on an A320 remains during engine shutdown and startup. No preflight cockpit checks address spoiler position, so if one step is missed on after landing flow maximum brakes will engage when rejected takeoff is armed for takeoff.

Narrative: ON TAXI OUT FROM CLE WHEN AUTOBRAKES WERE ARMED, MAX BRAKES ENGAGED.
