Narrative:

A prisoner was boarded on our flight. I believed the captain had the armed escort form because I saw him receive a form from a station person. The form he received was actually a hazmat form for something else. During the flight, I asked flight attendant about the prisoner. The captain said 'what prisoner?' we then discovered that we had a prisoner without the proper forms on board. This was about 1 hour into the flight. If I had known that the captain had not received the forms in oakland, I would have spoken up. The prisoner escorts said that they had filled out forms at the gate, but then the agent tore them up.

Title: MD80 FO RPT ON THE BOARDING OF A PRISONER WITH AN ARMED ESCORT WITHOUT THE REQUIRED DOCUMENTS AT OAK, CA.

Narrative: A PRISONER WAS BOARDED ON OUR FLT. I BELIEVED THE CAPT HAD THE ARMED ESCORT FORM BECAUSE I SAW HIM RECEIVE A FORM FROM A STATION PERSON. THE FORM HE RECEIVED WAS ACTUALLY A HAZMAT FORM FOR SOMETHING ELSE. DURING THE FLT, I ASKED FLT ATTENDANT ABOUT THE PRISONER. THE CAPT SAID 'WHAT PRISONER?' WE THEN DISCOVERED THAT WE HAD A PRISONER WITHOUT THE PROPER FORMS ON BOARD. THIS WAS ABOUT 1 HR INTO THE FLT. IF I HAD KNOWN THAT THE CAPT HAD NOT RECEIVED THE FORMS IN OAKLAND, I WOULD HAVE SPOKEN UP. THE PRISONER ESCORTS SAID THAT THEY HAD FILLED OUT FORMS AT THE GATE, BUT THEN THE AGENT TORE THEM UP.
