Narrative:

Flight planned and dispatched for ship X, flight xyz. The flight plan and wdr showed ship X, flight xyz. Anc loaded and fueled ship Y for flight xyz an error. The crew accepted ship Y for flight xyz and did not discover their error until over yvr. They notified dispatcher mr X who had taken the flight from me. He verified that the wrong aircraft had been used on flight xyz. I was notified of the error dec/wed/01, XA00Z.

Narrative: FLT PLANNED AND DISPATCHED FOR SHIP X, FLT XYZ. THE FLT PLAN AND WDR SHOWED SHIP X, FLT XYZ. ANC LOADED AND FUELED SHIP Y FOR FLT XYZ AN ERROR.
