Narrative:

Glen rose into dfw and first officer had glen rose 5 plate out-of-date. Captain had glen rose 6, but by mistake, had pulled out dal plate which is also called glen rose 6. Out of fuz, first officer asked to see new plate again and concurred with the turn to the east and did not notice it was for dal field. Approach control called and vectored us back on course. Aside from the obvious mistakes, these could have been prevented by not having 2 different procedures named the same thing.

Title: MD80 CREW HAD A TRACK DEV ARRIVING AT DFW.

Narrative: GLEN ROSE INTO DFW AND FO HAD GLEN ROSE 5 PLATE OUT-OF-DATE. CAPT HAD GLEN ROSE 6, BUT BY MISTAKE, HAD PULLED OUT DAL PLATE WHICH IS ALSO CALLED GLEN ROSE 6. OUT OF FUZ, FO ASKED TO SEE NEW PLATE AGAIN AND CONCURRED WITH THE TURN TO THE E AND DID NOT NOTICE IT WAS FOR DAL FIELD. APCH CTL CALLED AND VECTORED US BACK ON COURSE. ASIDE FROM THE OBVIOUS MISTAKES, THESE COULD HAVE BEEN PREVENTED BY NOT HAVING 2 DIFFERENT PROCS NAMED THE SAME THING.
