Narrative:
Aircraft at departure called maintenance with equipment cooling valve light and EICAS message. Dispatch aircraft per MEL with equipment cooling system in standby and outbleed valve breaker P13 pulled as per procedure in MEL. Aircraft landed in slc with this breaker another 922 pulled and collared with numerous write ups for instrument blocking overheat. Unknown how other breaker was pulled but witnessed that aircraft was dispatched with only the one called out on MEL was pulled.

Title: A B767-300 WAS DISPATCHED IN NON COMPLIANCE WITH THE EQUIP COOLING FAN DEFERRED AS INOP PER THE MEL BUT SPECIAL PROCS WERE NOT ACCOMPLISHED CORRECTLY.
Narrative: ACFT AT DEP CALLED MAINT WITH EQUIP COOLING VALVE LIGHT AND EICAS MSG. DISPATCH ACFT PER MEL WITH EQUIP COOLING SYS IN STANDBY AND OUTBLEED VALVE BREAKER P13 PULLED AS PER PROC IN MEL. ACFT LANDED IN SLC WITH THIS BREAKER ANOTHER 922 PULLED AND COLLARED WITH NUMEROUS WRITE UPS FOR INSTRUMENT BLOCKING OVERHEAT. UNKNOWN HOW OTHER BREAKER WAS PULLED BUT WITNESSED THAT ACFT WAS DISPATCHED WITH ONLY THE ONE CALLED OUT ON MEL WAS PULLED.
