Narrative:

Air carrier X was northeast bna departure climbing to FL290 requesting FL410. Air carrier Y was an ord inbound at FL310, who had been on a vector and routed back over iiu. The okk fix was in the flight plan, but not issued. Air carrier X was issued a turn to clear and climb away from air carrier Y. When air carrier Y passed iiu and turned for the next fix (vhp -- not okk) that is what created the loss of separation.

Narrative: ACR X WAS NE BNA DEP CLBING TO FL290 REQUESTING FL410. ACR Y WAS AN ORD INBOUND AT FL310, WHO HAD BEEN ON A VECTOR AND ROUTED BACK OVER IIU. THE OKK FIX WAS IN THE FLT PLAN, BUT NOT ISSUED. ACR X WAS ISSUED A TURN TO CLR AND CLB AWAY FROM ACR Y. WHEN ACR Y PASSED IIU AND TURNED FOR THE NEXT FIX (VHP -- NOT OKK) THAT IS WHAT CREATED THE LOSS OF SEPARATION.
