Narrative:
After arrival at ZZZ1, passenger was looking for her electric wheelchair that she gate checked in ZZZ. First officer went downstairs and saw wheelchair in forward cargo with battery terminals still connected and we had no form listing hazmat loaded. Location and disposition must be given to flight crew prior to departure.

Title: AN MD80 WAS DISPATCHED IN NON COMPLIANCE WITH NTSB HAZMAT REGS BY LOADING AN ELECTRICAL WHEELCHAIR WITH THE BATTERY CONNECTED AND NO PAPERWORK.
Narrative: AFTER ARR AT ZZZ1, PAX WAS LOOKING FOR HER ELECTRIC WHEELCHAIR THAT SHE GATE CHKED IN ZZZ. FO WENT DOWNSTAIRS AND SAW WHEELCHAIR IN FORWARD CARGO WITH BATTERY TERMINALS STILL CONNECTED AND WE HAD NO FORM LISTING HAZMAT LOADED. LOCATION AND DISPOSITION MUST BE GIVEN TO FLC PRIOR TO DEP.
