Narrative:
This was day 2 of a very difficult 4 day flight. Not sure what first officer's motivation was. He was a former USAF instructor. Was out on disability last yr from airline until nov. Been on plane 1 yr. Unlike the MD88, there is usually only 1 or 2 ways to program FMS or work autoplt. This guy went out of his way to develop new ways and adamantly push them. Made being PNF difficult, in that your monitoring task got difficult. On this occasion the STAR was not in FMS. In past, everyone builds STAR (3 points, VOR, designated waypoint, arbitrary waypoint beyond to track radial). STAR calls for left turn to intercept and stay clear of island. Pilots usually monitor raw data and stand ready to fly manually. This first officer did not want to build STAR. I did and he took offense. He wanted to fly 'ground track' to intercept. (Checked later with B757 instructor. The navigation choices are heading select, heading hold or LNAV to verify what I thought.) selected LNAV at 400 ft expecting him to fly his manual departure with FMS backup. He then mumbled something on departure and I thought he said he wasn't getting a signal. Checked his radio and I, at first, mistakenly put in sed 116.8 vice pdx 111.8 frequency (did from memory) but it had been an XA00 body time arrival for short layover. He tuned it back, then intercepted back to right. At level off, gave me a lecture about not 'respecting him' and listed off 6 things I had done that he had written down. I am going to talk to professional standards. This guy gets lost in minutia but doesn't do basic procedures in a timely manner. Did not know proper procedure for trimming aircraft. And it went downhill from there.
