Narrative:
After entering russian airspace en route nrt, 200 NM west of liski intersection, passenger had epileptic seizure. Emt onboard examined patient. Phone patched with medical clinic through HF radio in cockpit. Determined patient must be on ground and receive medical attention as soon as possible. Company dispatch concluded divert to fai, ak, necessary. Route being flown was A218. Left turn of 180N degrees to a south offset 25 NM completed, descended 500 ft. Several attempts made to contact russian ATC schmidta, due to extreme difficulty of both crew and ATC controller unable to understand each other due to language barrier, not sure if russian ATC understood or cleared the flight to divert. Proceeded along offset until in united states airspace. Contacted anc control and proceeded to fai. Patient delivered to medical care. Flight was dispatched to nrt without problem. Note: company has no guidance or specific procedures regarding divert in russian airspace.
