Narrative:

On may/fri/03, flight was pushed from gate at mke, 8 mins early. Mke WX was reported as 230 degrees/11 KTS 9 SM scattered 10000 ft, overcast 15000 ft altimeter 29.67. As per company policy, both pilots had mke 10-9 airport diagram displayed on map holder. We received the following taxi instructions: 'taxi to runway 25L, via txwys B, K, and runway 13/31, cross runways 19L/1R, 19R/1L' and begun our taxi. This was the first flight of the day for the aircraft, which also had 2 performance limiting MEL's. During the taxi, the captain was taxiing, the first officer was receiving performance numbers from load planning, performing the 'taxi' and 'before takeoff' checklists, and double-checking our performance data, along with MEL limitations. The taxi proceeded normally on txwys B and K, and runway 13/31, with extreme vigilance crossing runways. As we approached the approach end of runway 31, we were told by ground control to stop. Ground control informed us we had taxied approximately 400 ft past the hold short lines for runway 25L. It is important to note: 1) we were still on runway 13/31, 2) runway 13/31 has a displaced threshold approximately 500 ft from approach end of runway 31, 3) txwys M, north separate runway 31 from runway 25L, 4) apparently the 'hold short' line for runway 25L is on runway 31 in the displaced threshold zone. No traffic conflict, or any other abnormal condition or action happened as a result of crossing the hold short line. We were given takeoff clearance, and the flight proceeded normal, without incident. Contributing factors: 1) not expecting a hold short line on a separate runway separated by txwys. 2) high cockpit workload, related to MEL/performance limiting items. 3) nothing on 10-9 chart , or surface chart display to indicate abnormal location of hold short lines. Corrective action: 1) note on 10-9 (airport diagram charts) for abnormal hold short locations. 2) a simple taxi route/instruction from ATC, ie...in the past normally given taxi to runway 25L via taxiway M cross runway 19R/left. Supplemental information from acn 583256: on may/fri/03, flight from mke to ord. The first officer contacted ground control to taxi. Ground control instructed us to taxi via txwys B, K, runway 31 to runway 25L. On runway 31 we were instructed to monitor the tower frequency. The first officer's head was down running the before takeoff checklist and figuring out the takeoff performance numbers. The aircraft's engine speed switches, which control engine overspd, were MEL'ed. This MEL required the apr (automatic power reserve) switch to be off. This MEL also required the thrust reverser not to be used above 30% N1, so more consideration was being given to aircraft takeoff performance. The hold short line for runway 25L is on runway 31. The captain taxied across the hold short line approximately 2 aircraft lengths past and stopped short of taxiway M, still on runway 31. The tower controller advised us that we had taxied past the hold short line. I then told the tower that we were ready for takeoff. About 10 seconds later he cleared us for takeoff. We finished the before takeoff checklist and took off with no further incident. Contributing human factors were the pilots were not expecting the hold short lines for runway 25L to be on runway 31 and the first officer's head being down dealing with the extra performance consideration due to the engine speed control switches being MEL'ed.

WE RECEIVED THE FOLLOWING TAXI INSTRUCTIONS: 'TAXI TO RWY 25L, VIA TXWYS B, K, AND RWY 13/31, CROSS RWYS 19L/1R, 19R/1L' AND BEGUN OUR TAXI. THIS WAS THE FIRST FLT OF THE DAY FOR THE ACFT, WHICH ALSO HAD 2 PERFORMANCE LIMITING MEL'S. I THEN TOLD THE TWR THAT WE WERE READY FOR TKOF. ABOUT 10 SECONDS LATER HE CLRED US FOR TKOF. WE FINISHED THE BEFORE TKOF CHKLIST AND TOOK OFF WITH NO FURTHER INCIDENT. CONTRIBUTING HUMAN FACTORS WERE THE PLTS WERE NOT EXPECTING THE HOLD SHORT LINES FOR RWY 25L TO BE ON RWY 31 AND THE FO'S HEAD BEING DOWN DEALING WITH THE EXTRA PERFORMANCE CONSIDERATION DUE TO THE ENG SPD CTL SWITCHES BEING MEL'ED.
