Narrative:
At XA30 I ate a salmon dinner packed separately in first class oven. At approximately XD00 I became violently ill with vomiting/diarrhea lasting through the remainder of flight. I was taken to hospital via ambulance. Upon landing in new york, I was hospitalized for 3 days for food poisoning.

Narrative: AT XA30 I ATE A SALMON DINNER PACKED SEPARATELY IN FIRST CLASS OVEN. AT APPROX XD00 I BECAME VIOLENTLY ILL WITH VOMITING/DIARRHEA LASTING THROUGH THE REMAINDER OF FLT. I WAS TAKEN TO HOSPITAL VIA AMBULANCE. UPON LNDG IN NEW YORK, I WAS HOSPITALIZED FOR 3 DAYS FOR FOOD POISONING.
