Narrative:
C172 was issued a 275 degree heading, climb to 3000 ft, contact departure. The AT72 was well past the C172 then issued a 290 degree heading, contact departure. The 2 headings were written on the wrong flight plans. Separation was lost.

Title: IND LCL CTLR ISSUES TKOF CLRNC TO AN AT72 FOLLOWING A C172 WHICH RESULTS IN A LOSS OF SEPARATION AFTER XFER TO DEP CTL. WRONG DEP HDGS WRITTEN AND XMITTED TO WRONG ACFT.
Narrative: C172 WAS ISSUED A 275 DEG HDG, CLB TO 3000 FT, CONTACT DEP. THE AT72 WAS WELL PAST THE C172 THEN ISSUED A 290 DEG HDG, CONTACT DEP. THE 2 HDGS WERE WRITTEN ON THE WRONG FLT PLANS. SEPARATION WAS LOST.
