Narrative:
We were #1 for takeoff at holding point for runway 9L. I had made the prepare for takeoff PA when we were #3 for takeoff and we were awaiting clearance onto the runway. The cabin to cockpit call chime rang and the purser spoke to the relief pilot relating that there had been some problem with the video briefing and she would be making an additional PA after takeoff. I asked if it was okay to go and the answer was, 'yes.' we were subsequently cleared into position and I chimed the cabin for takeoff. After reaching cruise altitude, several of the other flight attendants notified the cockpit crew that we had taken off without the critical safety briefing items being completed. I was never told by the purser that the required items had not been completed or that they needed more time to do so. Had I been told that more of the briefing needed to be accomplished, I would not have taken off. There was obviously a poor communication from the purser to the flight deck relating what was really occurring with the safety video and we should have been told that more time was needed to complete the safety briefing. Supplemental information from acn 617036: we briefly discussed that we were aware there are some things in the passenger briefing that can be delayed after takeoff. We did 'assume' by her statement of 'ok to go' that it seemed most of the brief was done and then the audio visual broke. We assumed she was going to finish any non-critical items right after takeoff, so we would not interrupt with any PA's or phone calls until they were completed. None of the critical items were briefed prior to takeoff. Supplemental information from acn 617040: the purser was totally unaware of the importance of this procedure. I found this strange since she has been flying 42 yrs. Supplemental information from acn 618867: flight attendants were in process of getting our equipment to perform the safety demonstration manually. The purser made a PA stating 'a manual safety demonstration would be demonstrated after takeoff.' I called the purser and told her we couldn't do that. It had to be done before takeoff. She said something to the effect that we were getting ready to take the runway and what did I want her to do? I admit I was stunned. There was a miscom between cockpit and the purser. The cockpit did not know it was the safety demonstration that was not performed.

Title: A B777 TAKES OFF WITHOUT THE CABIN ATTENDANTS BEING ABLE TO COMPLETE THEIR REQUIRED SAFETY ITEMS BRIEFING DUE TO A BROKEN VIDEO WHEN THE PURSER MISCOMMUNICATES WITH THE FLT CREW JUST BEFORE TKOF FROM SBGR, FO.
Narrative: WE WERE #1 FOR TKOF AT HOLDING POINT FOR RWY 9L. I HAD MADE THE PREPARE FOR TKOF PA WHEN WE WERE #3 FOR TKOF AND WE WERE AWAITING CLRNC ONTO THE RWY. THE CABIN TO COCKPIT CALL CHIME RANG AND THE PURSER SPOKE TO THE RELIEF PLT RELATING THAT THERE HAD BEEN SOME PROB WITH THE VIDEO BRIEFING AND SHE WOULD BE MAKING AN ADDITIONAL PA AFTER TKOF. I ASKED IF IT WAS OKAY TO GO AND THE ANSWER WAS, 'YES.' WE WERE SUBSEQUENTLY CLRED INTO POS AND I CHIMED THE CABIN FOR TKOF. AFTER REACHING CRUISE ALT, SEVERAL OF THE OTHER FLT ATTENDANTS NOTIFIED THE COCKPIT CREW THAT WE HAD TAKEN OFF WITHOUT THE CRITICAL SAFETY BRIEFING ITEMS BEING COMPLETED. THERE WAS OBVIOUSLY A POOR COM FROM THE PURSER TO THE FLT DECK RELATING WHAT WAS REALLY OCCURRING WITH THE SAFETY VIDEO AND WE SHOULD HAVE BEEN TOLD THAT MORE TIME WAS NEEDED TO COMPLETE THE SAFETY BRIEFING. SUPPLEMENTAL INFO FROM ACN 617036: WE BRIEFLY DISCUSSED THAT WE WERE AWARE THERE ARE SOME THINGS IN THE PAX BRIEFING THAT CAN BE DELAYED AFTER TKOF. WE DID 'ASSUME' BY HER STATEMENT OF 'OK TO GO' THAT IT SEEMED MOST OF THE BRIEF WAS DONE AND THEN THE AUDIO VISUAL BROKE. WE ASSUMED SHE WAS GOING TO FINISH ANY NON-CRITICAL ITEMS RIGHT AFTER TKOF, SO WE WOULD NOT INTERRUPT WITH ANY PA'S OR PHONE CALLS UNTIL THEY WERE COMPLETED. NONE OF THE CRITICAL ITEMS WERE BRIEFED PRIOR TO TKOF. SUPPLEMENTAL INFO FROM ACN 617040: THE PURSER WAS TOTALLY UNAWARE OF THE IMPORTANCE OF THIS PROC. I FOUND THIS STRANGE SINCE SHE HAS BEEN FLYING 42 YRS. SUPPLEMENTAL INFO FROM ACN 618867: FLT ATTENDANTS WERE IN PROCESS OF GETTING OUR EQUIP TO PERFORM THE SAFETY DEMONSTRATION MANUALLY. THE PURSER MADE A PA STATING 'A MANUAL SAFETY DEMO WOULD BE DEMONSTRATED AFTER TKOF.' I CALLED THE PURSER AND TOLD HER WE COULDN'T DO THAT. IT HAD TO BE DONE BEFORE TKOF. SHE SAID SOMETHING TO THE EFFECT THAT WE WERE GETTING READY TO TAKE THE RWY AND WHAT DID I WANT HER TO DO? I ADMIT I WAS STUNNED. THERE WAS A MISCOM BTWN COCKPIT AND THE PURSER. THE COCKPIT DID NOT KNOW IT WAS THE SAFETY DEMO THAT WAS NOT PERFORMED.
