Narrative:
I was flying the ILS runway 9R approach to ord. At the OM, the captain (PNF) switched from approach control to tower and received landing clearance for runway 9R. The captain then completed the remaining items on the before landing checklist. At 500 ft AGL, the captain made the required callout of sink rate and approach speed. He then switched the active communication frequency from tower to ground control prior to landing. The communication radio has a dual head with both an active and inactive side. The inactive side has a red bar across the frequency window. The captain had intended to place the ground control frequency in the inactive side, but instead, switched the active tower frequency to ground control prior to landing. After the landing roll, I relinquished control of the aircraft to the captain. It was at this time I realized that we were no longer on the tower frequency. After clearing the runway onto a taxiway, ground control cleared us to the gate. There were no traffic conflicts and no comment from ground control. I believe this was a result of fatigue. The captain commutes. His home is a 3 hour commute. Report time for this trip was early morning. The captain probably had, at most, 4 hours sleep the night before, a 3 hour drive, and a 12 hour trip prior to this event.

Title: AN APCHING, LNDG, MD80 PIC CHANGES THE VHF TO GND CTL FREQ AT 500 FT AGL PRIOR TO LNDG AT ORD, IL.
Narrative: I WAS FLYING THE ILS RWY 9R APCH TO ORD. AT THE OM, THE CAPT (PNF) SWITCHED FROM APCH CTL TO TWR AND RECEIVED LNDG CLRNC FOR RWY 9R.
