Narrative:
Ok takeoff; after rotation; noticed odor. Flight attendants called and reported smoke in the cabin. Donned oxygen masks immediately. Leveled off at 3000 ft MSL. Coordinated emergency return to ZZZ. Callback conversation with reporter revealed the following information: reporter stated that at the gate; maintenance determined that the pack had failed causing the smell of oil and burnt fiberglas throughout the aircraft. Supplemental information from acn 659092: made box pattern to runway 34R; declared an emergency; completed all applicable checklists; contacted operations; made PA to passenger that we would be returning to ZZZ. Made overweight landing. Taxied to gate. Deplaned passenger. Maintenance logbook completed. Debriefed flight attendants at gate.

Title: MD80 CREW EXECUTED AN EMER RETURN TO THE DEP ARPT FOLLOWING AN UNUSUAL SMELL IN COCKPIT AND SMOKE IN THE CABIN.
Narrative: OK TKOF; AFTER ROTATION; NOTICED ODOR. FLT ATTENDANTS CALLED AND RPTED SMOKE IN THE CABIN. DONNED OXYGEN MASKS IMMEDIATELY. LEVELED OFF AT 3000 FT MSL. COORDINATED EMER RETURN TO ZZZ. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: RPTR STATED THAT AT THE GATE; MAINT DETERMINED THAT THE PACK HAD FAILED CAUSING THE SMELL OF OIL AND BURNT FIBERGLAS THROUGHOUT THE ACFT.
