Narrative:
Part number XXX was sent to maintenance on apr/xx/02 and returned to air carrier as part number XXX on may sun/02. Repair authority/authorized paperwork was correct showing the part as an XXX. However; the shop order was per an xxy. Blade was inspected back from repair authority/authorized in shop and part number on original shop order was modified by someone; no initials or employee number adding -001. The engine was removed on dec/mon/05 as part on normal maintenance. The part number and company part number did not match the shop order. Callback conversation with reporter revealed the following information: the reporter stated this event occurred about 2 yrs ago and was just discovered on a paperwork audit accomplished by the reporter. The engine was removed and the incorrect blade removed and replaced. No engine damage was incurred.

Title: A JT8D ENG WAS OVERHAULED AND RETURNED TO SVC AND INSTALLED ON AN ACFT WITH 1 INCORRECT FAN BLADE INSTALLED. THIS WAS AN ENG SHOP ERROR.
Narrative: PART NUMBER XXX WAS SENT TO MAINT ON APR/XX/02 AND RETURNED TO ACR AS PART NUMBER XXX ON MAY SUN/02. REPAIR AUTH PAPERWORK WAS CORRECT SHOWING THE PART AS AN XXX. HOWEVER; THE SHOP ORDER WAS PER AN XXY. BLADE WAS INSPECTED BACK FROM REPAIR AUTH IN SHOP AND PART NUMBER ON ORIGINAL SHOP ORDER WAS MODIFIED BY SOMEONE; NO INITIALS OR EMPLOYEE NUMBER ADDING -001. THE ENG WAS REMOVED ON DEC/MON/05 AS PART ON NORMAL MAINT. THE PART NUMBER AND COMPANY PART NUMBER DID NOT MATCH THE SHOP ORDER. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: THE RPTR STATED THIS EVENT OCCURRED ABOUT 2 YRS AGO AND WAS JUST DISCOVERED ON A PAPERWORK AUDIT ACCOMPLISHED BY THE RPTR. THE ENG WAS REMOVED AND THE INCORRECT BLADE REMOVED AND REPLACED. NO ENG DAMAGE WAS INCURRED.
