Narrative:
I had surgery on my eye muscles on feb/mon/06. Was cleared to return to work on feb/tue/06. On mar/thu/06 I was jacking the left main landing gear of aircraft at night. In the wind; the aircraft fell off the jack and broke the lower strut service port on the left main landing gear.
