Narrative:
After pushback; received salute and release. Captain completed after start flow with 6 pack recall. 6 pack recall illuminated doors. Looked at overhead door lights and all lights indicated out. Did light test and aft cargo lights burnt out. Changed lights in aft cargo lights indicator and lights indicated on. At this time ramp personnel came out waving hands pointing toward aft cargo door. Captain shut down #2 engine. Ramp closed aft cargo door; light went out. Again received salute and release. Talked to operations taxiing and they said ramp forgot to close aft door. Breakdown in ramp SOP.

Narrative: AFTER PUSHBACK; RECEIVED SALUTE AND RELEASE. CAPT COMPLETED AFTER START FLOW WITH 6 PACK RECALL. 6 PACK RECALL ILLUMINATED DOORS.
