Narrative:
I put our aircraft on an active runway with the flaps up. In 33 yrs of flying airplanes this is a first for me! I attribute it to a failure of leadership (mine); a failure of checklist discipline (mine); and an untimely interruption in our checklist flows. After pushback and release of the pushback crew we accomplished our flows up to the point of 'standing by flaps' (first officer); at which time mdw ground called us and said; 'with your direction of flight; you may have runway 31C or runway 4R.' instead of me asking him to 'stand by;' the first officer and I discussed it briefly and chose runway 4R because of an 8 KT tailwind on runway 31C. We got taxi clearance to runway 4R. My mind must have jumped a cog or something because 'flaps 5;' 'control check;' and 'before taxi checklist' was never called for by me. Additionally; I also failed to confirm flaps on the confign check flow and check the takeoff warning horn after pushing the attend button on the 'before takeoff checklist.' when the takeoff warning horn sounded on the runway; I told tower we needed to exit the runway. We never set takeoff thrust and ground speed was never over 10-15 KTS. We re-entered the sequence for takeoff and the rest was uneventful. An irregularity report was submitted by me.

Title: A B737-300 ON TKOF ROLL HAD TKOF WARNING HORN. REJECTED TKOF AT 15 KTS. FOUND FLAPS NOT SET FOR TKOF.
Narrative: I PUT OUR ACFT ON AN ACTIVE RWY WITH THE FLAPS UP. IN 33 YRS OF FLYING AIRPLANES THIS IS A FIRST FOR ME! I ATTRIBUTE IT TO A FAILURE OF LEADERSHIP (MINE); A FAILURE OF CHKLIST DISCIPLINE (MINE); AND AN UNTIMELY INTERRUPTION IN OUR CHKLIST FLOWS. AFTER PUSHBACK AND RELEASE OF THE PUSHBACK CREW WE ACCOMPLISHED OUR FLOWS UP TO THE POINT OF 'STANDING BY FLAPS' (FO); AT WHICH TIME MDW GND CALLED US AND SAID; 'WITH YOUR DIRECTION OF FLT; YOU MAY HAVE RWY 31C OR RWY 4R.' INSTEAD OF ME ASKING HIM TO 'STAND BY;' THE FO AND I DISCUSSED IT BRIEFLY AND CHOSE RWY 4R BECAUSE OF AN 8 KT TAILWIND ON RWY 31C. WE GOT TAXI CLRNC TO RWY 4R. MY MIND MUST HAVE JUMPED A COG OR SOMETHING BECAUSE 'FLAPS 5;' 'CTL CHK;' AND 'BEFORE TAXI CHKLIST' WAS NEVER CALLED FOR BY ME. ADDITIONALLY; I ALSO FAILED TO CONFIRM FLAPS ON THE CONFIGN CHK FLOW AND CHK THE TKOF WARNING HORN AFTER PUSHING THE ATTEND BUTTON ON THE 'BEFORE TKOF CHKLIST.' WHEN THE TKOF WARNING HORN SOUNDED ON THE RWY; I TOLD TWR WE NEEDED TO EXIT THE RWY. WE NEVER SET TKOF THRUST AND GND SPD WAS NEVER OVER 10-15 KTS. WE RE-ENTERED THE SEQUENCE FOR TKOF AND THE REST WAS UNEVENTFUL. AN IRREGULARITY RPT WAS SUBMITTED BY ME.
