Narrative:

We pushed back with an experienced crew. We stopped; paused; then pulled forward. This action was repeated several times. We called to the pushback crew to see if everything was ok. They responded that the were having trouble disconnecting the tow bar. There was a pause. In my peripheral view I saw some movement; and when I looked down I noticed the parking brake was not set; nor do I recall the pushback crew ever asking for the brakes to be set. Just then the ground crew called 'brakes.' we heard some movement from the forward galley. We received a call from the flight attendants in the back and we called the forward galley. In the meantime; the ground crew was trying to talk to us on the interphone. It took some time to communicate what had happened. The ground crew told us everything was ok and just drove away. Upon landing at destination; we contacted maintenance and inspected the aircraft. Supplemental information from acn 695414: started both engines during the push back. Some distance from the gate the aircraft was rocked back and forth by the tug in an attempt to disconnect the tow bar. As soon as the tow bar was disconnected the aircraft rolled forward; hitting the tug with the nose cone. The ground crew then asked us to set the parking brake. Ground crew assured us everything was ok.

Title: B737 FLT CREW HAS ACFT ROLL FORWARD DURING PUSHBACK AND CONTACTS PUSHBACK TUG AT KOA.

Narrative: WE PUSHED BACK WITH AN EXPERIENCED CREW. WE STOPPED; PAUSED; THEN PULLED FORWARD. THIS ACTION WAS REPEATED SEVERAL TIMES. WE CALLED TO THE PUSHBACK CREW TO SEE IF EVERYTHING WAS OK. THEY RESPONDED THAT THE WERE HAVING TROUBLE DISCONNECTING THE TOW BAR. THERE WAS A PAUSE. IN MY PERIPHERAL VIEW I SAW SOME MOVEMENT; AND WHEN I LOOKED DOWN I NOTICED THE PARKING BRAKE WAS NOT SET; NOR DO I RECALL THE PUSHBACK CREW EVER ASKING FOR THE BRAKES TO BE SET. JUST THEN THE GND CREW CALLED 'BRAKES.' WE HEARD SOME MOVEMENT FROM THE FORWARD GALLEY. WE RECEIVED A CALL FROM THE FLT ATTENDANTS IN THE BACK AND WE CALLED THE FORWARD GALLEY. IN THE MEANTIME; THE GND CREW WAS TRYING TO TALK TO US ON THE INTERPHONE. IT TOOK SOME TIME TO COMMUNICATE WHAT HAD HAPPENED. THE GND CREW TOLD US EVERYTHING WAS OK AND JUST DROVE AWAY. AS SOON AS THE TOW BAR WAS DISCONNECTED THE ACFT ROLLED FORWARD; HITTING THE TUG WITH THE NOSE CONE. THE GND CREW THEN ASKED US TO SET THE PARKING BRAKE. GND CREW ASSURED US EVERYTHING WAS OK.
