Narrative:
We went on a field trip to ZZZ from ZZZ1. Drove up to ZZZ to remove and install a new L2 door slide. We arrived at ZZZ XA30 on may/tue/06. When we remove the serviceable slide from shipping crate we discovered that the back shell was cracked and broken on the corner of the shipping safety pin. I called my supervisor and told him we could not install this slide because of the damage. He called me back after he talked to the maintenance controller on duty. We were instructed to come back to ZZZ1 and bring the damaged slide with us and remove one from aircraft Y because no other slide was available until XC45 from ZZZ2 to ZZZ1. We were instructed to drive back to ZZZ and install the slide on aircraft X and return that aircraft to service. We arrived back in ZZZ at XA15 and were escorted to aircraft X and installed it on the aircraft. When I signed off the aircraft logbook it slipped my mind to sign an rii in the logbook. We returned back to ZZZ1 with a girt bar that needed to go on the slide on aircraft Y to return that one to service. My mind on the way back to ZZZ was on the girt bar and arriving in time to ZZZ1 to not delay aircraft on gate X for its flight. When I discovered we did not leave the girt bar at ZZZ1 and I called my supervisor and informed him of the dilemma; halfway to ZZZ. When I got to work I informed my supervisor of my discovery and told him it needed an rii signature after I confirmed it in the gpm. Contributing factors: my mind was preoccupied with trying to keep aircraft in service when we had both parts in ZZZ and the other mechanic with me forgot also about the rii in the logbook. We both inspected the final installation; but forgot about the rii on the paperwork.

Title: AN MD80 HAD THE L #2 DOOR ESCAPE SLIDE REPLACED; BUT THE FAA REQUIRED INSPECTION WAS NOT ACCOMPLISHED.
Narrative: WE WENT ON A FIELD TRIP TO ZZZ FROM ZZZ1. I CALLED MY SUPVR AND TOLD HIM WE COULD NOT INSTALL THIS SLIDE BECAUSE OF THE DAMAGE. HE CALLED ME BACK AFTER HE TALKED TO THE MAINT CTLR ON DUTY. WE WERE INSTRUCTED TO COME BACK TO ZZZ1 AND BRING THE DAMAGED SLIDE WITH US AND REMOVE ONE FROM ACFT Y BECAUSE NO OTHER SLIDE WAS AVAILABLE UNTIL XC45 FROM ZZZ2 TO ZZZ1. WE WERE INSTRUCTED TO DRIVE BACK TO ZZZ AND INSTALL THE SLIDE ON ACFT X AND RETURN THAT ACFT TO SVC. WE ARRIVED BACK IN ZZZ AT XA15 AND WERE ESCORTED TO ACFT X AND INSTALLED IT ON THE ACFT. WHEN I SIGNED OFF THE ACFT LOGBOOK IT SLIPPED MY MIND TO SIGN AN RII IN THE LOGBOOK. WE RETURNED BACK TO ZZZ1 WITH A GIRT BAR THAT NEEDED TO GO ON THE SLIDE ON ACFT Y TO RETURN THAT ONE TO SVC. MY MIND ON THE WAY BACK TO ZZZ WAS ON THE GIRT BAR AND ARRIVING IN TIME TO ZZZ1 TO NOT DELAY ACFT ON GATE X FOR ITS FLT. WHEN I DISCOVERED WE DID NOT LEAVE THE GIRT BAR AT ZZZ1 AND I CALLED MY SUPVR AND INFORMED HIM OF THE DILEMMA; HALFWAY TO ZZZ. WHEN I GOT TO WORK I INFORMED MY SUPVR OF MY DISCOVERY AND TOLD HIM IT NEEDED AN RII SIGNATURE AFTER I CONFIRMED IT IN THE GPM. CONTRIBUTING FACTORS: MY MIND WAS PREOCCUPIED WITH TRYING TO KEEP ACFT IN SVC WHEN WE HAD BOTH PARTS IN ZZZ AND THE OTHER MECH WITH ME FORGOT ALSO ABOUT THE RII IN THE LOGBOOK. WE BOTH INSPECTED THE FINAL INSTALLATION; BUT FORGOT ABOUT THE RII ON THE PAPERWORK.
