Narrative:
Departing mdw on midway 4 departure; crew put 4000 ft in altitude window when clearance had been to 3000 ft. We climbed to 4000 ft and were then informed we should be at 3000 ft but to stay at 4000 ft. We had an XA30 show this morning and it was my 4TH before XC00 in a row. I believe the loop error in the cockpit was caused by fatigue.
