Narrative:

I taxied aircraft X from the south side of the ramp to runway 26L via txwys Y1; Y; and M. Moments later; the local controller observed an AT72 taxiing on runway 26L brought my attention to it. Upon the aircraft exiting runway 26L and joining taxiway M; I asked aircraft X if he was established on taxiway M. He replied in the affirmative. Apparently; aircraft X entered runway 26L at M4; realized the mistake and immediately exited the runway. Numerous times during this yr; aircraft have mistakenly entered runway 26L at M4 without clearance. This is an extremely dangerous event and regardless of all the preventive measures taken recently by the airport authority/authorized and ATC; it still occurs. Taxiway M4 is rarely used by aircraft entering or exiting runway 26L/8R. Also; controllers do not use M4 to cross runway 26L/8R. This taxiway should be closed and removed since it does not provide any operational advantage to the airport and is apparently an extreme hazard. Supplemental information from acn 796111: taxi out of remote south side ATC instructed runway 26L Y1 M runway 26L coming out of Y right turn to M taxiway. Had completed taxi checklist before taxi onto runway 26L notice our mistake checked runway for aircraft and exited immediately on first exit.

Title: MIA GND CTLR DESCRIBED RWY INCURSION WHEN ACFT TAXIING TO RWY 26L ENTERED RWY AT TXWY M4; CTLR ALLEGING MULTIPLE SAME TYPE EVENTS.

Narrative: I TAXIED ACFT X FROM THE S SIDE OF THE RAMP TO RWY 26L VIA TXWYS Y1; Y; AND M. MOMENTS LATER; THE LCL CTLR OBSERVED AN AT72 TAXIING ON RWY 26L BROUGHT MY ATTN TO IT. UPON THE ACFT EXITING RWY 26L AND JOINING TXWY M; I ASKED ACFT X IF HE WAS ESTABLISHED ON TXWY M. HE REPLIED IN THE AFFIRMATIVE. APPARENTLY; ACFT X ENTERED RWY 26L AT M4; REALIZED THE MISTAKE AND IMMEDIATELY EXITED THE RWY. NUMEROUS TIMES DURING THIS YR; ACFT HAVE MISTAKENLY ENTERED RWY 26L AT M4 WITHOUT CLRNC. THIS IS AN EXTREMELY DANGEROUS EVENT AND REGARDLESS OF ALL THE PREVENTIVE MEASURES TAKEN RECENTLY BY THE ARPT AUTH AND ATC; IT STILL OCCURS.
