Narrative:

While working the radar position; I had coordinated with ZAU ARTCC to descend aircraft X to FL240 immediately clearing traffic. Within 30 seconds of that coordination; my d-side (assistant) took a call from ZAU ARTCC to change the plan to stop aircraft X at FL260. My d-side then told me to 'stop that aircraft at FL260.' the wrong aircraft was stopped and aircraft X descended to FL240 and eventually creating an operational error with a crj that was climbing the opposite direction. Both aircraft responded to TCAS advisories.
