Narrative:

I as the pilot flying; the first officer is not familiar with teb operation and we have not flown a lot lately. We reviews the runway 24 takeoff procedure during the departure brief with 'special emphasis' due to the number of errors with this by others at teb. The teb 5 procedure was loaded in to the FMS. We are efb equipped and must stow for takeoff and landing so the (teb 5) was not on the yoke. We discussed the clearance 'teb 5 radar vectors white.' the FMS shows 'white; but not the col routing as shown on the chart. We were assigned 12000 ft and a heading shortly after takeoff. Then we were cleared direct col as depicted 'white.' we both questioned 'col' as it was not on our route of flight and queried departure. He said it should be found on the teb 5 SID. We did not see it on the FMS display so I told the first officer to look at the chart on the efb. He found it and entered it for FMS direct. My headset lo/hi switch was in low which deactivates air warnings and I didn't hear the alt alert during the excursion to 12900 ft. The switch was later discovered in the flight and moved to high. The controller initially said to descend back to 12000 ft; but immediately changed it to climb to 14000 ft and turn right. By now we had found the 'as depicted' routing and understood the confusion. A more complete review of the chart should have been accomplished to include the routing block. This is different from the 'as depicted' clearance. Perhaps another line item should be added 'white -- col as depicted.' in the future I will brief the first officer we may get col as depicted and have it tuned in the raw data VHF navigation and review the SID 'completely. I will also be more 'attentive' to altitude control regardless of distractions.
