Narrative:
Extremely noisy cockpit. ATC radio calls missed and difficult to hear checklist items and responses. Cockpit airflow is not muffled and the high decibel environment is distracting and fatiguing. Promises were made by equipment manager about 1 year ago that this issue would be fixed with boeing support. To this date I suspect that no modifications have been done on any of the 757 fleet. The fatigue level is so great that it has a direct impact on safety. When the affected aircraft are flown on ETOPS routes longer than 5 hrs; a relief pilot should be scheduled to allow some 'decompression' from the noisy and fatiguing cockpit environment.

Title: An air carrier Captain reported the noise level is so high in the B757 cockpit that it is fatiguing and distracting. He stated action was promised a year ago but still no progress noted.
