Narrative:

On a routine flight from ord to buf we experienced a simultaneous shutter of the airframe and 'bang' on climb out passing through 16000' over lake michigan. After noticing the left engine EPR at 0, the captain notified ATC of the emergency and requested return to ord and called upon me for the severe engine damage checklist. As the captain flew the aircraft, handled ATC communications, and briefed the cabin crew, I performed the checklist approved by his verification. After the severe engine damage checklist was completed, we performed the single engine approach and landing checklist along with a review of the emergency landing evacuate/evacuation checklist. Following completion of normal checklists, the captain made an uneventful landing on runway 27R at ord and ordered emergency trucks to standby and assess the situation after rollout. After verifying no abnormalities, the captain proceeded to the gate. The initial diagnosis was internal turbine wheel damage aft of the hot section.

Title: ENGINE FAILURE ON CLIMBOUT, RETURN LAND.

Narrative: ON A ROUTINE FLT FROM ORD TO BUF WE EXPERIENCED A SIMULTANEOUS SHUTTER OF THE AIRFRAME AND 'BANG' ON CLBOUT PASSING THROUGH 16000' OVER LAKE MICHIGAN. AFTER NOTICING THE LEFT ENG EPR AT 0, THE CAPT NOTIFIED ATC OF THE EMER AND REQUESTED RETURN TO ORD AND CALLED UPON ME FOR THE SEVERE ENG DAMAGE CHKLIST. AS THE CAPT FLEW THE ACFT, HANDLED ATC COMS, AND BRIEFED THE CABIN CREW, I PERFORMED THE CHKLIST APPROVED BY HIS VERIFICATION. AFTER THE SEVERE ENG DAMAGE CHKLIST WAS COMPLETED, WE PERFORMED THE SINGLE ENG APCH AND LNDG CHKLIST ALONG WITH A REVIEW OF THE EMER LNDG EVAC CHKLIST. FOLLOWING COMPLETION OF NORMAL CHKLISTS, THE CAPT MADE AN UNEVENTFUL LNDG ON RWY 27R AT ORD AND ORDERED EMER TRUCKS TO STANDBY AND ASSESS THE SITUATION AFTER ROLLOUT. AFTER VERIFYING NO ABNORMALITIES, THE CAPT PROCEEDED TO THE GATE. THE INITIAL DIAGNOSIS WAS INTERNAL TURBINE WHEEL DAMAGE AFT OF THE HOT SECTION.
