Narrative:
An inspection was performed on a pilot trainer (pt) far 135 helicopter. Copies of the manufacturers inspection form are placed in a workbook for completion by the mechanics. This inspection consisted of a 'before first flight; after last flight'; 30 hour; 100 hour and 200 hour inspection. The 200 hour inspection consists of three items and two were not applicable; by the aircraft configuration. The third is an inspection of the 'swashplate'. The log entry has the 200 hour inspection entered and complied with. The manufactures 'inspection form page;' with the work reference and completion block for the mechanics signature; was removed and discarded; and not in the final work package. There is no way to verify if the inspection was completed and by whom. Failure to follow the manufacturers inspection procedure and a misleading/incorrect log entry are the result.
