Narrative:
Norcal initial approach stated; 'expect the tipp toe visual to runway 28L.' the final approach controller stated; 'cleared for the visual approach;' versus 'cleared for the tipp toe visual approach to runway 28L.' I was surprised to receive this changed clearance. We started to turn and descended toward the airport. A few moments later the controller seemed to be confused by our altitude and position. The final controller then told us; 'cross menlo at 4000 ft.' I stated; we understood that we were cleared for the visual approach.' at that point we had already started to execute our own visual approach versus the 'tipp toe visual runway 28L;' per his instructions. I think the controller then realized the 'tipp toe' omission. He appeared to be apologetic. He then sent us to the sfo tower frequency.
