Narrative:
Takeoff from runway 22L. Set power to 1.4 with brakes applied as aircraft rolled. Asked for auto throttle; no engagement; set power manually. Take off roll long rotated with about 2500 ft remaining. Aircraft sluggish in the climb; gear up; half rate climb power. First officer noted the tri (thrust rating indicator) was already in climb power. At cruise discussed what happened. During before start checklist read maximum takeoff looked at tri; minds eye said takeoff. In reality it was still in cl (climb). For me it was a slight fatigue problem and poor checklist discipline during that phase of the preparation to leave.
