Narrative:
After block-out; we requested and received a maintenance deferral for a write-up for no start on engine 2; ignition B (ignition a normal). We recorded the MEL; maintenance item and authorization numbers in the logbook while at the hold line for runway 35L. Not realizing that further action was required; I inadvertently departed without doing so.
