Narrative:
Around two hours into the flight I noticed that the trim wheel was moving in a jerking motion rather than the smooth or fluid motion. Also the aircraft began to ossulate up and down between 250-300 FPM on the vsi. The autopilot was holding altitude within +/- 100 ft. Up until this point the trim system worked normally. After talking with maintenance control and our dispatcher; myself; the first officer and the dispatcher agreed the safest action was to divert. Maintenance control advised us that the aircraft had a history of trim problems; and on one test flight they received a jammed stab. However the aircraft was fixed and returned to service.

Title: A319 Captain reports trim wheel moving in a jerky fashion in response to autopilot trim inputs. Aircraft has a history of trim problems and crew elected to divert to a suitable airport for maintenance.
