Narrative:
Right outer brake was replaced prior to takeoff. Normal taxi out. During takeoff roll aircraft was vibrating. Approaching 100 KTS it became more pronounced and by rotation speed it was very uncomfortable. We talked to maintenance and dispatch and decided that we would have equipment standing by at our destination for the landing. On landing the first officer set aircraft down on left main. It was smooth. He then lowered the right main to the runway and the vibrations were once again very uncomfortable. We used thrust reverse only and used the brakes approaching 40 KTS where vibrations were close to normal. We taxied to the gate and debriefed all involved; maintenance; operations; oncoming crew; emergency crew.

Title: After having a right outer brake replaced; a noticeable and uncomfortable vibration occurred on takeoff. An emergency was declared at the destination and the vibration occurred again after touchdown but disappeared at around 40 KTS.
