Narrative:
Air carrier X slc-phx scheduled departure runway 34R at slc XX42 mdt. An small aircraft Y did not hear the tower give him any departure heading or turn or instructions to contact departure control. We were cleared into position to hold and last observed the small aircraft Y climbing straight out off the departure end. We anticipated he would be given a turn or that we would be cleared to maintain visibility contact with him. When we were cleared for takeoff neither was mentioned in our takeoff clearance. We were instructed to turn left to a heading of 280 degrees. We began a turn to 280 degrees prior to reaching the end of runway 34R at an altitude of approximately 500' AGL. When already established in our left turn the tower controller again repeated, 'air carrier X turn left to heading 280.' we acknowledged that we were in the turn and asked if they wanted us to switch to departure. Tower did not acknowledge our transmission. At this point while the first officer was switching to departure I looked up and saw the small aircraft Y slightly right at the same altitude out our right front windscreen. (We were approaching from almost his 6 O'clock position). I yelled to my first officer and increased my left bank to 45 degrees and my first officer estimated he passed off our right wing tip at less than 50'. We are both convinced that we would have hit him had we not taken evasive action. At no time was traffic called out to us. No mention was made on tower or departure of the incident.

Title: ACR CAME IN CLOSE PROX TO AN SMA AFTER DEP.
Narrative: ACR X SLC-PHX SCHEDULED DEP RWY 34R AT SLC XX42 MDT. AN SMA Y DID NOT HEAR THE TWR GIVE HIM ANY DEP HDG OR TURN OR INSTRUCTIONS TO CONTACT DEP CTL. WE WERE CLRED INTO POS TO HOLD AND LAST OBSERVED THE SMA Y CLBING STRAIGHT OUT OFF THE DEP END. WE ANTICIPATED HE WOULD BE GIVEN A TURN OR THAT WE WOULD BE CLRED TO MAINTAIN VIS CONTACT WITH HIM. WHEN WE WERE CLRED FOR TKOF NEITHER WAS MENTIONED IN OUR TKOF CLRNC. WE WERE INSTRUCTED TO TURN LEFT TO A HDG OF 280 DEGS. WE BEGAN A TURN TO 280 DEGS PRIOR TO REACHING THE END OF RWY 34R AT AN ALT OF APPROX 500' AGL. WHEN ALREADY ESTABLISHED IN OUR LEFT TURN THE TWR CTLR AGAIN REPEATED, 'ACR X TURN LEFT TO HDG 280.' WE ACKNOWLEDGED THAT WE WERE IN THE TURN AND ASKED IF THEY WANTED US TO SWITCH TO DEP. TWR DID NOT ACKNOWLEDGE OUR XMISSION. AT THIS POINT WHILE THE F/O WAS SWITCHING TO DEP I LOOKED UP AND SAW THE SMA Y SLIGHTLY RIGHT AT THE SAME ALT OUT OUR RIGHT FRONT WINDSCREEN. (WE WERE APCHING FROM ALMOST HIS 6 O'CLOCK POS). I YELLED TO MY F/O AND INCREASED MY LEFT BANK TO 45 DEGS AND MY F/O ESTIMATED HE PASSED OFF OUR RIGHT WING TIP AT LESS THAN 50'. WE ARE BOTH CONVINCED THAT WE WOULD HAVE HIT HIM HAD WE NOT TAKEN EVASIVE ACTION. AT NO TIME WAS TFC CALLED OUT TO US. NO MENTION WAS MADE ON TWR OR DEP OF THE INCIDENT.
